Itunes, the software, is free. The Apple Itunes store sells music tracks (and videos) that can be played on an Apple IPod. A small percentage of the tracks can be played on other MP3 players, but most are in a format that can only be played via ITunes or on Apple IPod.
You do need to sign up with ITunes music store to buy music. When I did it I provided a credit card number. I don't know if this is required if you only plan on using ITune cards. Either way, if you buy no songs there is no charge. They would just have credit card info on hand if you want to buy on credit.
The ITunes cards are used to add credit to your account which you can use to pay for music (or videos).

Yea, I figured it out after I posted this. Duh! Sometimes I think the blonde hair dye seeps into my brain!!
