Huh? They wanted how much you made at your other clubs, and info like who you live with? I hope it's a nice place. I'd feel weird putting that stuff on file.
The most info I've ever given is never more than is listed on my driver's license.

I've only ever had to give them a copy of my driver's license. If a club asked me to fill out a form like that, I'm not sure I'd do it.
Wow. Never had to fill out any of that, just proof of age.
All I've been asked for is proof of age (they photo copied it). One club I work for just looked at me, said "You're over 21?" And I said "Yes" and he said "OK".





at my current club i had to give 2 forms of ID, fill out the "general info" paper (address/phone/etc), fill out a clothing size/height/weight/etc paper, and sign the "private contractor" agreement (states you are not an employee, etc.)
the only lengthy thing i had to fill out was the dj card. every girl at my club has to have one on file in the dj booth. it's basically a 8X8 card that has what bands/songs/music you like, as well as your hobbies, where you are from, any "sexy" info.. like if you've been in playboy or something like that.
it's mostly so if we get a new dj or you work on a different shift as normal the dj has a quick ref to look at.
i'm pretty sure the clothing/bra size thing is for promos. occasionally if we send girls to be "ring girls" at a fight or to promote at some other similar event, they'll get them all the same outfit.

i wanted to add that, from my experience at least, the clubs i've worked at which asked for the least amount of info were the worst/dirtiest i've worked for. the more the club asked for, the better it was (like my current). i'm not saying that's true for all clubs.. but for around here it seems to be.
course, i've never been asked what i used to make at old clubs, cept maybe during the interview as in "what are you used to/expecting to make?". i don't see the point in asking you to put it on paper.
The places I've worked have never been divey but they never asked for anything more than one form of id and a few signatures. I don't know why they wanted you to fill out what you made at other places, I don't think that relevant at all.





Nobody else had to give them your social?
I had to fill out a W-2 (even though I'm a dancer???) and emergency contact and they photocopied my social and D.L.

They also asked if I had worked at any other clubs, but I hadn't.
For me it has depended on where I was at. When I started at my club here in my town, I filled out like a basic job application (name, address, social security#, etc....) and they copied my license. In Vegas I did an application at one club that was 20 pages long (it took forvever) It had things like weight, height, measurements, hair color, tattoo locations and descriptions...but I guess that's just Vegas being Vegas.

Are you sure it wasn't just a generic employment application? Those often ask for your last three places of employment and salary.

Most in-depth was a half sheet card that I listed height, weight, address, age, hair/eye color on
Least was at a place that asked me how old I was (I was underage and lied) and threw me on stage about 10 min later to start.





I had to do basic job app stuff... name, DOB, address. They took my picture and scanned my ID. If I can recall correctly, every club I've worked at has been that way.
