Wear something really loose on the day of your shoot, and no underwear. If you have lines on your skin from tight clothes, it can take ages for them to go away. Take your make-up and some stuff to touch up your hair, and remember that the camera tends to make you looks paler so doing your make-up a little heavier than usual can help. I'd say 4 outfits and a dress are enough - did your photog say how many outfits you could shoot? Most have a set number of "looks" that is included in the price.
Also, think about how you want your photo's to look. Do you want to show your face? If not, take care to do poses where your hair covers your face, or where your head can be cropped off, blocked by props etc. Are you paying for retouching or anything?
