It's proof of eligibility to work. They won't run a search on it.

They obviously will know your real name, and sometimes ask for DOB. Every club I ever worked for had my real name on file. This is normal. Other than that, they will not run a background check. That only happens when you get a dance license.

No. Once again, it's proof of eligibility to work. Copies are easy to forge.

Actually, a legal requirement of the new US Obamacare law is that businesses must report to the IRS via form 1099 all moneys paid out to 'independent contractors' in excess of $400 per year. This legal requirement was supposed to take effect in 2012 but was postponed for a year by the US congress. So far no effort has been made in Washington to postpone it again ... which means that if clubs will be required to issue 1099's to the IRS in the spring of 2013, they need to be tracking money paid out to 'independent contractor' dancers throughout 2012. Form 1099 requires the 'independent contractor's real name, real address, and real SS#.

also, kind of off topic... for any of u ladies who work or have worked at any deja vu clubs i'm sure ya'll are familiar with the looooowwww dance prices (ie. $100-150 for 30min vips etc) and to top it off the 40% they take...so dancers are doing $20 dances for $12, and vips for like $96 or less; how in the HECK do dancers put up w that????? and make money???? i calculated and wowwwww that's shitty; i know many ppl told me well deja vu has alot of guys/customers because of the low dance prices; so in other words dancers (have to) work twice as hard??? ahm deja vu seems to attract alooot of cheap etc customers idk

Regarding eligibility to work forms: Employers are required (as of 1986) to fill out a I-9 form and keep it on file for every person they hire. They have to sign that they've witnessed and checked the physical ID and that it belongs to the person being hired. Acceptable ID's are a driver's license AND a Social Secuity card, a passport, or a military ID.

Employers must keep all the forms on file for three years and can receive a spot inspection at any time. If they don't have the documentation, they may be hit with heavy fines. I remember a popular restaurant being penalized for this several years back. So, your club is trying to follow the rules and protect themselves.
