Never made one, but this's how I'd do it..get some stretchy spandex fabrici, measure your thigh area, add an inch or so (may have to play around with that) allowing for hems top & bottom, get a piece of elastic, do a basting stitch to hold in place in the middle cover it w/the spandex, sew over hems, then sew over basting stiches in the middle.
Does this make sense? I could show you better than tell it..also if you had a serger or access to one, that'd be ideal.
They have some not too expensive ones at Lover's Lane, (don't know about on their website).

To make a garter you want to buy elastic and ribbon. The ribbon will need to be at least triple the measurement of your thigh. The spools you can pick up in the notions section of Joann or Hancock work well. Cut the elastic to ~2 inches shorter than your thigh measurement (you need it tighter so that it can stay up). Put a few stitches down and then grab the thread at the begging of the stitching and pull it as you sew. You HAVE to do this or the elastic won't feed through. Stretch the elastic as you sew so that when the garter is relaxed the ribbon bunches. To get a nice gather, stretch out your elastic as far as you can and cut ribbon to match. Elastic will curl when relaxed so make sure it isn't twisted when you connect it. If you don't have a serger just lay the end over and sew it flat.
