I do spend money somewhat on food (higher quality food), but otherwise I just don't spend. I've been able to pay off a lot of things this way, as well as pay for dental work, surgery, new car, etc that I needed. Just pretend you never made it.
Also, you may be interested in a credit card called Care Credit to pay for medical expenses. Some doctors take it and payment plans run from 0mo - 18mo, but it might be incentive for you to work and pay it off? If you think it won't be, don't do it!
Years ago, I used to listen to Suze Orman audiobooks on the way to&from school, and I learned a lot of her methods. Hers are more for like, paying off debt though. She says to use the snowball effect and stockpile all your money toward the smallest debt. Then when thats paid off, stockpile it all to the next smallest, etc. Till you have like no debt.
In terms of saving, maybe go the investment route? I don't know much about investing other than Roth IRAs, but its next on my list to learn! There are lots of good books out there on amazon or your local library!
