Look for a body cream with hyaluronic acid in it. It is normally found in the dermis layer of our skin, and provides elasticity and lubrication. Because it also attracts 1000 times its weight in water, it really helps alleviate dry skin. Your skin is crying out for moisture, not grease---skip the cocoa butter for now.
Conditioning the skin with oil is most effective when the moisture level of the skin is elevated---you could also try rubbing on a layer of pure, unrefined shea butter when you are out of the shower in the steamy bathroom--effectively sealing in the moisture you just absorbed from the shower.

Short term: Hydrocortosone cream for irritation. Lanacane for itching relief
    Longer term: Borage oil

jojoba oil works too because it closely mimics the body's own natural oils.

I used to have this problem, it was easilly solved by drinking 2 litres of water a day! (but remember to take sea salt too - 1/4 of a teaspoon daily I think is about right. SEA SALT not table salt, the are not the same!)

Yes, drink more water, take oil pills like salmon oil & primrose oil, and bathe with oatmeal powder.
Moisturizer every day, if you're able to.

All these suggestions are good . . . Also, one of my friends, a Floridian transplanted to Texas (!) bought a tiny water fountain -- one of those decorative things you put in your house -- and it releases the water into the air, working as a humidifier. (Needs daily refills, to give you a good idea of how much water winds up in the air!.)

See a dermatologist. You may have eczema.
