    Default removal of old implants

    1. If I have my implants done through the arm pit and in a few years I decide to go bigger how do they remove the old implants and put the bigger inplants in?
    2. How is a implant removed that was done in the arm pit that may have leaked and become deflated?


    Some of the girls say everything is done throuh the arm pit and some say they cut a very large incision under the breast. What in the hell is the point of having implants put in throught the arm pit if you have to take them out a different way if they deflate. Then you have two scars and it seems so dumb. If anyone knows the answer to my first two questions please fill me in.

    I am not positive, but according to my surgeon, yes it is often necessary to make a second set of incisions under the breasts to remove or change implants. That is just one of the reasons that my surgeon prefers to do breast augmentations using an inframammary(under each breast, at the crease) incisions. Also, there are less safety risks when you go inframammary instead of transaxillary(through the armpit). For example, the armpits have plenty of sweat glands that potentially contain bacteria. An armpit incision can expose this bacteria and resultingly the bacteria can slow the incision's healing process. The armpit is also filled with a plethora of blood vessels, which can also interfere with the healing process. Furthermore, surgeons generally have more controlled placement of the implants when going through the breast crease than through the armpits, which often results in more natural looking work when performed through the crease. If you need to change or remove implants, going through the inframammary crease will be much easier because there will already be a scar to cut through, and it will be in a convenient place. Maybe some surgeons CAN change implants through the transaxillary scars(I do not know), but I doubt that the implants' placements will be as controlled or as good as through inframammary incisions. Overall, there are less potential complications if you go inframammary.

    Implants have to come out inframammary because it's the only way to make a big enough incision. Saline implants are filled once the are in place, so that's why the incision doens't have to be that big for putting them in.

    Research has shown that going in IF means more chance of an infection. Contamination of the implant is far more likely through the breast ducts than the armpit. And this is means another breast surgery. A keloid in the armpit is not a big deal compared to capsular contracture, which is VERY rare with trans-ax incision. Also, you are more likely to ruin nerves to your breast tissue through IF, meaning loss of sensation to the breast.

    Trans-axillary (armpit) is the only way to get completely behind the muscle, which means less chance of capsular contracture and bottoming out. And my opinion is that breasts look more natural with the armpit incision because more of your natural tissue can be on top of the implant, smoothing it out. Less rippling and there is a natural "muscle" bra, which means less sagging later on. And less interferance when doing a mamogram.

    With that said, it is the most difficult placement, but it's important to find a surgeon that really knows how to do this. It's kind of moot for the OP at this point because ther eis already going to be an IF incision, but for first timers, trans-ax should be seriously considered. I'm not saying IF is bad, but any surgeon that uses it as his preferred placement (when it's not a revision surgery) because of potential contamination or better placement is not giving his patient the correct information. Maybe for HIM it's better because he doens't know how to to trans-ax, but it's not inherently better.

    Does anyone know if they can be deflated and then removed through the armpit??? I would hate to have a scar when i eventually have to have a redo.

    Edited to add: A friend of mine had hers put in through the armpit. The surgeon swore to her at the time that she could have them removed this way too. Anyway less than a year later she opted to have the implants removed and replaced with a different type and the same surgeon insisted that he never said such a thing at all, so she now has the two sets of scars. She actually went to a different surgeon for the second op as she was pretty pissed off with the first considering.
